Drone Flight Approval Request Form

All drone operations conducted over any University-owned property, or involving the use of a University-owned aircraft, must receive prior approval through the completion of this form.  

The request form requires the following information:

• Pilot Information including Part 107 License
• Certificate of Authority
• Copy of Aviation / Drone Liability Insurance Coverage
• Acknowledgement of FAA Commercial Operation Requirements
• Complete Flight Plan

Please submit the form at least two weeks in advance of your planned flight.

All University-owned drones must be insured through UNM Risk Services prior to flying.
